The Prescriptionease Program

New Patient Referrals

As of April 1, 2011 the PrescriptionEase Program will only accept new patient referrals from Mount Carmel entities (physicians, case managers, CNPs, nurses, etc.) and not directly from patients. Please use the Referral Form on this website to electronically submit your patient's referral for our immediate access.

Please note when you consider referring a new patient to our program, that we must strictly adhere to the patient eligibility requirements as stated on this webpage.


Most of the major pharmaceutical companies offer indigent programs known as patient assistance programs for eligible patients to receive their medications free of charge. The PrescriptionEase program offered by Mount Carmel, clears the roadblocks for Mount Carmel doctors and patients to gain access to and participate in these programs.

Patient Eligibility Reference Guide

  1. Patients must have an established Mount Carmel Doctor
  2. Patients must not have any prescription insurance coverage including Medicare D plans.
  3. A patient is not eligible if he/she is a veteran.
  4. Patient's household income level must be below federal poverty guideline levels and/or those levels determined by each specific patient assistance program.

The Average Income Levels of Most Programs Are:

  • Single household: under $21,660 a year
  • Married couples: under $29,140 a year

What PrescriptionEase Does For Patients:

  1. We screen and educate the patient and identify programs that they may be eligible for.
  2. Facilitate all of the paperwork required and submit the applications.
  3. Track all medications and provide follow up with refills and yearly reenrollments.
  4. Assist in medication compliance by assisting patients to participate in the various Patient Assistance Programs.

What PrescriptionEase Does For Doctors:

  1. We provide "ease" in their participation, by identifying the programs for their patients and providing all the required, completed paperwork.
  2. Assist in providing medication compliance for their patients
  3. Allows doctors more time with patients and less time on paperwork.
  4. Provides expedited Plavix process for indigent heart patients needing specific stent procedures ensuring patients an almost immediate 90 days supply. Follow up provides three more FREE, 90-day shipments.
